Urban Family Practice Received Target BP Award

October 24 - Urban Family Practice (UFP) is utilizing innovative techniques to treat high blood pressure, including a wellness program. Some participants’ blood pressure drops equal to the decrease seen in patients taking a drug that costs $280.00 a month.

The American Heart Association has awarded Urban Family Practice (UFP) a Target: BP™ award for their commitment to prioritize blood pressure control within Buffalo’s Lower West Side community.

UFP is recognized as a leader in the national effort to get people’s blood pressure under control and reduce the number of Americans who have heart attacks and strokes each year. Out of 1,674 health care organizations enrolled in Target: BP, UFP was one of only 438 nationwide to achieve gold status for controlled blood pressure rates for over 70% of its patients. UFP used YourCare Health Plan data to track patients with high blood pressure from April 2018 – June 2018. In April 2018, only of 50% of YourCare patients at UFP had normal blood pressure (lower than 140/90). By utilizing innovative techniques to treat high blood pressure, by the end of June 2018, 72% of UFP YourCare patients had normal blood pressure.

The innovative techniques UFP uses to treat high blood pressure are: Obtain a list of patients who have high blood pressure

  • Call the pharmacy for each patient with high blood pressure and send a 90-day supply order for the medication to lower blood pressure. This step enables the patient to start taking the medication even before his/her doctor’s appointment. A ninety-day supply of medication makes it easier for patients who may have a hard time getting to the pharmacy
  • Call the patient for an appointment
  • If his/her blood pressure is lower than 140/90, the patient is asked to return to the clinic monthly; if the patient’s blood pressure is over 140/90, weekly appointments are scheduled

Another innovative G-Health Enterprises program is the Wellness Program through the Greater Buffalo United Accountable Healthcare Network (GBUAHN). The program involves personal training, exercise classes, weight loss support groups, nutritional counseling and healthy cooking classes in their state-of-the-art gym and teaching kitchen.

Since its inception, the GBUAHN Wellness Program has reduced emergency room visits among its members by 67%, and in-patient hospitalizations by 80%. GBUAHN data across 18 months showed an average drop in blood pressure of 10 points systolic and 6 points diastolic. A three-month study of diabetic patients in the wellness program showed an average A1C drop of .6%. That level of a drop equals the decrease seen in patients taking a drug that costs $280.00 a month.

GBUAHN Awarded SOPHi Award

October 5 - GBUAHN was awarded a Spotlight on Population Health Award (SOPHi Award) for our emphasis on disease prevention and education, mental and emotional well-being, healthy eating, and policies and solutions to address socio-economic and cultural factors. The award was presented by Population Health Collaborative of WNY. This is the second year in a row that GBUAHN has been recognized with a SOPHi Award.

GBUACO’s 4th Annual Charity Golf Tournament

September 21 - GBUACO’s 4th Annual Charity Golf Tournament took place on Friday, September 21, 2018, at Glen Oak Golf Course in East Amherst. After hitting the links, golfers enjoyed an awards ceremony, basket raffle and check presentations to the charities that each received a donation from the tournament. Everyone enjoyed a live DJ, DJ Unknown. United Healthcare was the premier sponsor of this event.

Mayor Byron Brown’s Reading Rules Award Ceremony

August 25 - Mayor Byron Brown’s Reading Rules Award Ceremony (GBUAHN sponsored) Each summer students are encouraged to read a number of books (determined by grade level) and write about the books they’ve read. Participants who complete the challenge are invited to our awards ceremony at the end of the summer. At the event, students receive a certificate of achievement, souvenir t- shirt, book bag and a chance to win many great prizes.

Urban Family Practice Health Fair

July 21 - Thank you to the Westside Community of Buffalo for coming out to our celebration of “preserving good health” at the Urban Family Practice Health Fair. The Health Fair brought together families and neighbors in an attempt to heighten their awareness of vital health issues.

Our most recent Health Fair was held on Saturday, July 21st, 2018 from 10:00 am until 6:00 pm. The host and major sponsor for the Fair was The Raul & Toni Vazquez Foundation Inc, located at 564 Niagara Street, Buffalo, NY 14201.

The mission of The Hispanic Health Fair was to educate the community on matters of good health and risk factors for disease and was a wonderful success. During the event, Dr. Raul and Toni donated over 30 bikes to the community, bed in bags, bins with home goods and tablets. The fair saw over 1,000 people come in from the surrounding communities.

June 16 & 17 - The Juneteenth Festival of Buffalo was started in 1976 by B.U.I.L.D., a community-based organization once led by Toni Vazquez father William L. Gaiter, as a culturally relevant alternative to the country's Bicentennial Celebration. The group blocked off part of Jefferson Avenue -- the "Main Street" of Buffalo's Black community -- for a weekend celebration. Murals were painted and vendors set up booths to sell ethnic foods and wares -- entertainment and festivity were abundant. After several years, the Juneteenth Festival outgrew Jefferson Avenue. It is now held in Martin Luther King Jr. Park at the intersection of Best Street and Fillmore Avenue. Juneteenth Festival Inc. received 501(c) (3) status in 1985. In 2000, the name of the organization was changed to Juneteenth of Buffalo, Inc. to reflect its broader mission.

Juneteenth of Buffalo, Inc. exists exclusively for charitable and educational purposes. Staffed by volunteers including GBUAHN, its mission is to actively preserve and promote the broad spectrum of African American heritage through educational and cultural activities that will benefit the community as a whole.

JP Morgan Corporate Challenge with YourCare

June 14 - The J.P. Morgan Corporate Challenge Series is a world-wide series of 3.5-mile (5.6-km) running events open to groups of full-time employees from organizations within the business and public sectors. Participants may register only as part of a company team - no individual entries are accepted. After the race, each organization groups its participants into an unlimited number of four (4)-person teams in order to determine the race's top teams. Awards are presented for individual and team performances. Each company entered is led by a Company Captain, who creates the company team, recruits participants, and manages the team. Teams of competitive runners have a chance to qualify for the Series Championship, where they will compete against the champions from all the Series events for top honors in the corporate running community. Participating companies use the event as a platform to promote health and fitness in the workplace, foster goodwill and camaraderie among their employees, and show their commitment to the community and best environmental practices. The Corporate Challenge strives to make a difference through a donation to a not-for-profit organization in each host city, and through our operations initiatives designed to make the Corporate Challenge the world's greenest road race.
