The YourCare – G-HEALTH Partnership
On September 20, 2016, YourCare and GBUACO announced the formation of a historic healthcare partnership – a pilot program where YourCare became the first health plan providing Medicaid and other government-sponsored health insurance programs to pair up with a Medicaid accountable care organization (GBUACO) in New York State. Since then, the YourCare – G-HEALTH relationship has grown in fascinating ways, and we expect to finalize a contract extension in the coming weeks to solidify our partnership for another three years!
Overall, YourCare and G-HEALTH share one major thing in common – each organization’s mission encompasses their commitments to providing access to quality healthcare, removing barriers caused by social determinants of health, and improving health and happiness within the communities we serve for years to come.
The Buffalo Corporate Challenge is a tradition – meaning, there will be hundreds of local businesses also taking part in this friendly competition, and it is a wonderful way for us to show our commitment to G-HEALTH as a partner, and Western New York, as a local health plan in the community!

May 1 - Almost $1.5 million is coming to Buffalo from Albany to try and lift people out of poverty through better jobs. Lieutenant governor Kathy Hochul made that announcement Monday morning. Thrive Buffalo is one of the programs funded by this movement. Thrive is using this funding toward training people for careers where they're needed, then connecting those people with the employers that need them. What we want to do is make it easy for people who are already working, but they're underemployed, they're in financial hardship but they're in minimum wage jobs," she said. "We want to help them better connect to where these job opportunities are that are paying $4, $5, $6 more an hour." The United Way of Buffalo and Erie County, as well as the Community Foundation for a Greater Buffalo, is managing the money for the program, which is called Thrive Buffalo.
April 27 - We all strive to better ourselves in some capacity. Improve my health. Travel more. Manage my money better. Go back to school. Learn a hobby. And the list goes on. Join us as a source of renewed inspiration at this one-of-a-kind fair designed to help people get back on track. The goal is simple -- gather the most resources in one building for 7 hours to provide the tools for those in our community to achieve their resolutions before another year comes to an end. April 3 - YourCare Health Plan has renewed its contract with the Greater Buffalo United Accountable Care Organization, a value-based payment plan based in Buffalo.
The deal comes three years after the two organizations first partnered, following GBUACO’s creation as one of the first ACOs in New York’s program that rewards physicians and providers for cutting costs and improving care and outcomes for Medicaid recipients.
The organization, an affiliate of the Greater Buffalo United Accountable Healthcare Network (GBUAHN), also includes participation by providers at Erie County Medical Center, Kaleida Health, Jericho Road Community Health Center and the Greater Buffalo Independent Physicians Association.
GBUAHN, based in Buffalo, also operates a state-designated health home program that coordinates care for thousands of local individuals with Medicaid who have two or more chronic health conditions, a serious mental or behavioral health condition and/or HIV/AIDS.
That’s the same market served by YourCare, owned by Rochester-based Monroe Health Plan, which serves about 47,000 residents in metropolitan Buffalo and Rochester. The new contract will continue care for 9,000 members covered through the ACO, and allow room for more.
Dr. Raul Vazquez, CEO at GBUAHN and GBUACO, said the renewal of the contract with YourCare reflects an increased risk value of $750,000 and includes the addition of members with special needs. The new three-year contract also allows for telehealth services, allowing patients to receive care without leaving their homes and paying providers for providing that care.
“That’s going to be really important,” Vazquez said. “It’s in our vested interest to increase quality and reduce costs. We can do things differently in this region. These value-based agreements allow us to have a point of figuring out what we want, how much risk we’ll take on and what we need within the contract.”
The ACO is also finalizing details on another contract with a Medicaid/Medicare health plan provider that will add another 16,000 lives while offering an opportunity to expand into other parts of New York, he said. A decision is expected from the State Department of Financial Services later this month.
25 First Year UB Medical Students visited G-Health Enterprises
March 27 - On March 27th, 2019 over 25 First Year UB Medical Students visited G-Health Enterprises. As a part of their community enrichment course, the students were able to tour the building as well as meet several medical pioneers in the Medical Community. Dr. Kenneth Gayles, Dr. McDaniels, Dr. Raul Vazquez, and Pastor Pointer.
During their visit, students gathered in the William L. Gaiter conference room and heard about the start of African American and minority doctors in the Buffalo Hospital Systems. Dr. McDaniels told the stories of how he struggled to gain acceptance in the medical community and get the rights to practice within the hospital systems. Dr. Gayles was also able to dive into his personal experience receiving privileges to practice invasive cardiovascular treatments in local hospitals.
To conclude, Dr. Raul Vazquez was able to pour into the students the need for real interaction with patients. Vazquez was born in the South Bronx, New York to a single mother. He was raised with two younger brothers. Vazquez often accompanied his uncle on trips to the local hospital emergency room and felt distressed because neither the staff nor the physician could understand his uncle's Spanish. From that point, he'd aspired to become a doctor, and vowed to alleviate that barrier by becoming a doctor for the misunderstood and underrepresented people.
He went on to explain the need for social determinants of health and how for years he pioneered the local medical market, going beyond the examination room. He also explained to the students the need for better outcomes in patients, not just a quick 15-minute visit. With the future of healthcare leaning on VBP, doctors have to change their mentality on how they treat patients.
GBUAHN and YourCare Partner to Provide Warm Hats for Children
March 1 - GBUAHN and YourCare partnered to provide over 900 hats for every student at Frank A. Sedita School 30 on Buffalos lower west side. The goal was to help keep the young children warm and healthy to improve their attendance, continue great education and give back to the community — the hats were given to the students during their honor assembly. While seeing the joy the students had received their certificates and hats, Dr. Raul Vazquez, M.D. and his wife Toni Vazquez were moved to give each child with high-honor roll a $25.00 visa gift card.
February 20-21 - GBUAHN and YourCare team up in sponsoring the VBP Forward conference in Buffalo NY. VBP Forward is a conference dedicated to the next generation of value-based payment. As healthcare payment shifts to value, there are growing complexities to healthcare delivery and sustainability. This is most pronounced for high need and high-cost patients. VBP Forward is dedicated to service delivery institutions outside acute care offering immense value to patient outcomes. The conference is geared towards organizations that service Medicaid and Medicare special needs or complex populations or have an interest in that value chain. This inaugural event held in Buffalo, NY – with Jason Helgerson as the keynote speaker – will provide a roadmap for those organizations in their path towards effective VBP for special needs populations. The goal of the conference will be to provide both mature and nascent VBP organizations a roadmap for how to identify the need, develop value, contract on risk, and implement population health solutions.
